Birth certificate
The Embassy of Mongolia in Warsaw registers the birth of children born to Mongolian citizens residing in the Republic of Poland based on the request of the parents and issues a birth certificate.
A child must be registered within 30 days after birth.
If the child is being registered after 30 days, the parents must obtain an archive reference from the General Authority for State Registration confirming whether the child’s birth has been registered previously.
To register the child’s birth and obtain a birth certificate, the parents must prepare the following documents and appear in person at the Embassy of Mongolia in Warsaw:

A written request addressed to the Embassy from both parents regarding the child’s birth registration and issuance of a birth certificate (Both parents must sign the request and include their address and contact number in Poland.)

Medical certificate of birth (Polish birth certificate)

Child’s health booklet

Parents’ passports
-
Parents’ National ID cards (copies are acceptable)

Parents’ marriage certificate
If the parents have not registered or formalized their marriage, paternity determination procedures will apply. Based on the request of both parents, the child may be given either the mother’s or the father’s surname.
Important notice for families expecting a child:
The Law of Mongolia on Civil Registration and the updated “Regulation on Civil Registration” effective from 1 November 2018. Under the Regulation on Civil Registration, if a child is not registered within the legally required period (within 30 days of birth), the parents must additionally submit a “Statement of Unregistered Birth” along with the other required documents.
Furthermore, if the parents violate the Civil Registration Law, they must attach the decision of the state inspector imposing an administrative fine under the Article 15.24 of the Law on Violations of Mongolia.
Therefore, we kindly remind citizens to complete all registrations within the legal timeframe (within 30 days).
