Registration of Adoption

The Embassy of Mongolia in Warsaw shall register the adoption in accordance with the Article 16 of the Civil Registration Law of Mongolia. 
The adoptive parent shall submit the following documents to register the adoption of a child:
1.    Application for registration of the adoption of a child submitted to the Embassy of Mongolia in Warsaw /the application shall state the child’s surname, father’s name, first name, date of birth, birth or adoptive parents, guardians, adoptive parents’ surname, father’s name, first name, address of residence, and, if necessary, nationality/;

2.    The following documents of the adoptive parent:

•    Citizenship card (must be notarized),
•    Passport,
•    Marriage certificate if married;
•    Document proving residence in the Republic of Poland

3.    Birth certificate of the adopted child;
4.    A notarized copy of the identity card of the adoptive parent, guardian, or sponsor;
5.    A decision of the Soum or District Governor approving the adoption of the child (individual parent);
6.    A notarized copy of the parent’s consent and application for adoption submitted to the Soum or District Governor;
7.    A service fee of $10.
Note: The adoptive parent or legal guardian, or custodian must appear in person at the Embassy of Mongolia in Warsaw when registering the adoption.
